Grades 3-5 Fine Arts Course Selection Form

ONLY FOR STUDENTS IN GRADES 3-5

At the end of last school year, you were asked to complete the form below to indicate which Fine Arts courses your child would like to take for this school year.  To provide an additional chance for course selection, we are asking you to complete the information below and have your child return it to his/her homeroom teacher on August 23, 2011.  Every effort will be made to place your child in two of his/her top three choices. Students who have not returned a form will be placed in two classes.

Directions: Please number your child’s top three choices (example: 1, 2, and 3). Students will attend two of the classes listed below on alternating days.

_______ Visual Art: Students will learn about a variety of art, which includes pottery, mixed media,       

               drawing, painting and portfolio preparation.

_______ Vocal Music: Students will practice singing as an individual and as a group while learning 

                how to sing in tune and understand the expressive qualities of music.

_______ Physical Education/P.E./Movement: Students will participate in a variety of activities to   

               promote health and wellness as well as explore different individual team sports (this is in  

               addition to the physical activity they get each day).

_______ Dance: Students will learn a variety of dance styles such as ballet, jazz, tap, modern, and hip-

   hop.

_______ Drama: Students will learn acting, design, production, musical theatre, and clowning.

_______ Strings: Students will learn how to play a string instrument (e.g., violin, viola, cello).

_______ Band/Percussion: Students will learn how to play a band instrument (e.g., woodwind, brass 

               or percussion - drums cymbal, etc.)

_______ Piano: Students will learn the fundamentals of playing the piano.

_______ Television Production: Students will learn how to write, produce, videotape, and edit for a

                television broadcast .

_______ Digital Media: Students will create text and pictures, and audio and video to be stored and

               displayed through the computer.
